Phoenix Health Plan (PHP) is a contracted health plan with the Arizona Health Care Cost Containment System (AHCCCS). PHP services the following counties in the state of Arizona: Apache, Coconino, Gila, Maricopa, Mohave, Navajo, Pima, Pinal and Yavapai.

PHP is part of the Abrazo Health Care system, the second largest health care system in Arizona.

The Abrazo Health Care family includes six Arizona hospitals, including the Arizona Heart Hospital, the Arizona Heart Institute, Phoenix Health Plan and Abrazo Advantage Health Plan.

Phoenix Health Plan members can count on more than 8,500 physicians and providers, nearly 60 hospitals, urgent care centers, a 24-hour Nurse Advice Line and transportation services.
