Phoenix Health Plan is an AHCCCS-contracted health plan that has provided health care services to qualified KidsCare and AHCCCS families for over 25 years. Phoenix Health plan services the following counties in the state of Arizona: Apache, Coconino, Gila, Maricopa, Mohave, Navajo, Pima, Pinal and Yavapai.

Phoenix Health Plan is dedicated to serving the needs of our diverse member population. We have an extensive network of doctors and pharmacies throughout Arizona. In addition to receiving medical and dental coverage, all Phoenix Health Plan members have access to a 24-hour nurse line and transportation to appointments when needed. Our nurse line gives our members peace of mind by knowing they can call at any time to discuss a specific medical issue or ask general medical questions.

Mothers and mothers-to-be will receive the highest quality in service through our successful Obstetrics program. We offer incentives for early and regular prenatal care that extends through pregnancy and into a child's early years. We also offer a variety of other programs that are designed to improve the health of our members and their families.
