1. Apply for AHCCCS
- For most programs, you can receive an application at your local DES office. Visit www.azdes.gov for a listing of DES offices.
- If you meet the KidsCare guidelines, you can obtain an application through AHCCCS by clicking here. For more information you may also call KidsCare at:

2. Select the health plan that best suits you:
- AHCCCS contracts with various health plans to deliver the necessary medical services to eligible families.
- When you are filing out your AHCCCS application, you will be asked to select a health plan. It is up to you to decide which health plan you want for you and your family. Phoenix Health Plan will be listed as a health plan.
3. The approval process:
- Submit all requested documents to AHCCCS within the time frame they gave you.
AHCCCS will process your application and will send you one of the three following letters:
- Approval letter: You have coverage!
- Letter requiring more information: ACTION NEEDED! A quick response is needed within the time frame the letter states.
