Phoenix Health Plan is an AHCCCS contracted health plan that has provided health care services to qualifying AHCCCS and KidsCare families for over 25 years.

Phoenix Health Plan provides you with a large network of thousands of physicians and convenient hospitals, dentists, pharmacies, laboratories, specialists and transportation providers. You will find a dedicated staff who will deliver the highest quality customer service to you and your family.

Phoenix Health plan also provides a 24-hour nurse line (1-800-747-7997) to members so there will always be someone to answer any medical questions or concerns.

How to Obtain Health Care:

1. Apply for AHCCCS

For most programs, you can receive an application at your local DES office. Visit www.azdes.gov for a listing of DES offices.
If you meet the KidsCare guidelines, you can obtain an application at the Kids Care website, www.azkidscare.gov or visit its offices at:

2. Select the health plan that best suits you:

AHCCCS contracts with various health plans to deliver the necessary medical services to eligible families.
When you are filing out your AHCCCS application, you will be asked to select a health plan. It is up to you to decide which health plan you want for you and your family. Phoenix Health Plan will be listed as a health plan.

3. The approval process:

Submit all requested documents to AHCCCS within the timeframe they gave you.

AHCCCS will process your application and will send you one of the three following letters:

Approval letter: You have coverage!
Letter needing more information: ACTION NEEDED! A quick response is needed within the timeframe the letter states.
Denial Letter: For more information call AHCCCS at (602) 417-7100.
