Case Management

Phoenix Health Plan offers members the opportunity to participate in the Case Management Program when they become overwhelmed by multiple health problems. We are here to work with them to understand their illness and assist them to become self-sufficient. We will address their needs in a care plan and coordinate needed services with you. You may refer members to the program or they may refer themselves. Our services include:

Complex case management
HIV/AIDS
Transplants
OB high risk

Phoenix health plan also offers a Diabetes and Asthma Disease Management Program which focuses on education on the disease.
