History

Since 1983 Phoenix Health Plan (PHP) has provided Medicaid eligible members in Arizona access to a large network of medical and dental health care professionals.

For nearly 30 years, our Arizona neighbors in Maricopa, Gila and Pinal counties have counted on PHP. Since 2008, many residents living in Apache, Coconino, Mohave, Navajo, Pima and Yavapai counties are now part of the PHP family.

As of 2011 PHP provides more than 200,000 AHCCCS eligible residents with exceptional health care and customer service.

PHP is part of the Abrazo Health Care Family, which includes:

Six Arizona Hospitals.
The Arizona Heart Hospital.
The Arizona Heart Institute.
Urgent Care Centers.
Phoenix Health Plan and Abrazo Advantage Health Plan.

In 2006, we expanded into Medicare and began offering two Medicare Advantage Prescription Drug Plans. Serving 2,700 Arizona residents in Maricopa and Pinal counties, Abrazo Advantage (HMO) and Abrazo Advantage Plus (HMO SNP) Health Plans are both under contract with Medicare.

Our Abrazo Advantage Plus plan is specifically designed for dual-eligible beneficiaries, those eligible for Medicare and AHCCCS.
