Phoenix Health Plan has compiled a set of Practice Guidelines for your reference. These guidelines are developed through research and nationally accepted practices. They are based on valid and reliable clinical evidence or a consensus of healthcare professionals in the particular field. The guidelines are reviewed and updated periodically as appropriate, at a minimum of once annually. Additionally, Preventative Care Guidelines/Schedules have been made available for your reference.

These recommendations for care are suggested guides for making clinical decisions. Clinicians and their patients must work together to develop individual treatment plans that are tailored to the specific needs and circumstances of each patient.

For requests for training, obtaining additional information or receiving a hard copy of these guidelines mailed to your office, please contact network management department at 602-824-3700 or 800-747-7797.
