Learn How to Become a Participating Provider
When providers are contracted with Phoenix Health Plan, a Provider Service representative will be assigned to your practice and will serve as your primary point of contact. Your representative will visit you and your office staff on a regular basis to share important information and assist you with your administrative or operational concerns.
Screening Requirement:Important reminder, as a registered provider with the AHCCCS Administration, (Arizona's Medicaid Program), you are obligated under 42 C.F.R.100.1.1901(b), to screen all employees, contractors and/or subcontractors to determine whether any of them have been excluded from participation in Federal health care programs. You can search the HHS-OIG website, at no cost, by the names of any individuals or entities. The database is called LEIE, and can be accessed at http://www.oig.hhs.gov/fraud/exclusions.asp
Resources for seasonal flu virus:Below are resources for information on the H1N1 virus and the seasonal flu:
- The CDC has created a variety of resources on preventing the seasonal and H1N1 flu virus. Please click here for more information.
- Stop the Spread AZ is a campaign from Maricopa County Department of Public Health and provides information for the public on the flu virus.
- Mollen Immunization Clinics provides flu shots and is assisting schools with vaccinations through the Zap the Flu at Schools program. Click here for more information and for clinic locations.
Note on seasonal flu medicine: Tamiflu® is available through community pharmacies without prior authorization for children less than 6 y.o and for 6-18 year olds with current prescriptions for asthma inhalers.

In response to significant fiscal challenges facing the state and substantial recent growth in the Medicaid population, AHCCCS will implement several changes to the adult benefit package. On April 6, 2011 Governor Brewer signed Senate Bill 1619 which restores AHCCCS coverage of transplants that were eliminated for adults on October 1, 2010.
