Phoenix Health Plan members who choose to delay or prevent pregnancy may receive Voluntary Family Planning services as a covered benefit.

Covered services include
medical, surgical, pharmacological and laboratory services, contraceptive devices, as well as information and counseling necessary to allow members to make informed decisions regarding family planning methods.

 

No authorization is required for family planning services; however the diagnosis must indicate family planning.

Covered services include, but are not limited to, the following for male and female members:

At least annually, AHCCCS requires Phoenix Health Plan to communicate to members either in writing or verbally, the availability of family planning.

New PHP members receive a New Member Handbook upon enrollment.

2011 Fall Newsletter
PHP members receive quarterly PHP Newsletters. Family planning information is included in every fall edition.
