Smart Choices Club

Phoenix Health Plan cares about its members and wants them to be as healthy as possible. The Smart Choices Club encourages members to take control of their health. Through Smart Choices, members can get a Smart Choices Visa Card when they make specific well-care visits. This includes prenatal care visits for women in their first trimester, as well as well-woman visits and certain well-child visits.
