Phoenix Health Plan Transitioning to Care1st
On December 20, 2016, Phoenix Health Plan (PHP) entered into an agreement with Care1st Health Plan to transfer all members of Phoenix Health Plan to Care1st.
Arizona Health Care Cost Containment System (AHCCCS) has not yet approved this agreement, but the agreement is currently under review.

- Beginning October 1, 2016, Phoenix Health Plan will cover foot and ankle services performed by a licensed podiatrist, when the service is ordered by a primary care physician or primary care practitioner.

Protect Your Identity & Prevent Fraud
AHCCCS is adding pictures to its on-line verification tool that providers use to verify your coverage. If you have an Arizona driver’s license or state issued ID, AHCCCS will get your picture from the Arizona Department of Transportation Motor Vehicle Division (MVD). When providers pull up the AHCCCS eligibility verification screen, they will see your picture (if available) with your coverage details.
Prior Authorization Notice
Some medical services and prescription drugs require a prior authorization before members can receive them. Please check with your Primary Care Provider, who will request prior authorizations for you. The medical services and prescription drugs that require prior authorization may change at the start of or during the plan year.
